- Medical graduate degree (Apostilled).
- Mark sheets of the medical course (all terms).
- Medical Council registration certificate.
- Experience certificate (if any).
- Notary attested copies of the above documents.
- Twelve passport size photographs.
- Processing Fee.
- Medical fitness certificate with HIV report (could be submitted later).
- Valid Passport and two notary attested photocopies (could be submitted later).
